Position Overview

Ultimate Staffing Services is actively seeking a dedicated and compassionate Behavioral Health Services Personal Care Coordinator to join their client's team in California. This role focuses on supporting and coordinating internal and external resources for members referred to case management programs, ensuring compliance with all applicable state and federal regulatory requirements.

Responsibilities

  • Collaborate with case managers to assist members in navigating the healthcare delivery system and facilitate access to medical, psychosocial, and behavioral health benefits and services.
  • Monitor and respond to inbound case management inquiries and referrals, escalating to clinical staff as appropriate.
  • Provide outreach to members to facilitate timely completion of Health Risk Assessments via telephone, mail, or in-person as needed.
  • Support coordination of member care with PCP, Specialists, Behavioral Health, and Long Term Services providers to help members maintain functional independence.
  • Assist with coordinating the involvement of interdisciplinary care team members to implement individualized care plans and oversee related correspondence.
  • Ensure successful transitions of care for members moving between care settings by coordinating medical appointments and facilitating utilization review.
  • Follow UM policies for new authorization requests and manage data entry into authorization systems as needed.
  • Produce and distribute internal reports and follow established Health Services policies to address member and provider inquiries.
  • Maintain knowledge of current community resources to support case management goals.
  • Develop effective working relationships with stakeholders and assist with non-clinical training and onboarding of new employees.
  • Identify and report issues and trends to management, making recommendations to improve operations.
  • Collaborate on improvement efforts across departments regarding quality improvement projects and member satisfaction.
  • Participate in departmental meetings, training, and coaching sessions.
  • Understand Behavioral Health and 1115 Waiver programs and facilitate services as needed.
  • Perform other duties as required or assigned.

Qualifications

  • Bachelor's Degree in a health-related field or equivalent experience and training (Required).
  • Minimum three years of relevant experience in a healthcare or community setting (Required).
  • Valid California driver's license and acceptable driving record (Required).
  • Proficient in adapting to changing situations and efficiently alternating focus between tasks (Required).
  • Ability to meet accuracy and timeline requirements for regulatory compliance (Required).
  • Working knowledge of computer software, including Outlook, Word, Excel, and case management programs (Required).
  • Excellent communication skills and ability to maintain confidentiality (Required).
  • Knowledge of Medicare, Medi-Cal benefits, community resources, and medical terminology (Desired).

Required Work Hours

Monday to Friday, standard business hours.
